3. The forms will be submitted for approval by Gunn Admin, after which you will receive an email notifying you that you have been cleared to proceed with the next steps.

4. Once the Field Trip request has been approved, please download the PAUSD mandatory Extended Field Trip Form Packet (Forms A-H) from BOB and submit completed form packets to Linda Dillon at the D.O. Also provide a copy of the packets to Hindi Hipp.

NOTE: Please ONLY submit these forms to Linda Dillon AFTER you have received email approval from Gunn. (See step 3)

5. To protect against travel organizers who do not maintain a healthy financial position and adequate insurance, make sure that your travel agent is registered and that you record their registration number. You can verify registration by contacting the Office of the State’s Attorney General, Seller of Transportation Unit, 300 S. Spring Street, Los Angeles, CA 90013.

6. For international field trips, all participants are required to purchase the district's travel accident insurance. The rate is $3.20 per participant per trip day. Parents should write a separate check for insurance coverage, payable to PAUSD. Collect the checks and submit them to your school secretary or budget clerk for deposit. Provide the Risk Management office with a copy of the deposit slip so that they can purchase insurance.

7. If you will be driving a District vehicle into a foreign country (Mexico) as part of your field trip, additional auto coverage is required. Contact the Risk Management office at least one month in advance to obtain this insurance.  Note: Trips to Canada are not considered “foreign” for the purposes of driving a district vehicle.
